Job Description

Summary

The Licensed Vocational Nurse/LVN is a professional member of the healthcare team who provides skilled nursing care and prescribed medical treatment to patients in compliance with the Nurse Practice Act in the State of Texas. The LVN works under the direction of the supervising Nurse Practitioner or Physician.

Essential Duties and Responsibilities

  • Conducts individual patient assessment and interview and prioritizes the data collected to decide medical problem/condition and documents in a chart for the physician.
  • Obtains and records the patient’s vital signs and weight.
  • Aids physician with patient examination as needed.
  • Performs specific treatments as ordered by the physician and under the supervision of the Nurse Practitioner or Physician.
  • Reviews physician’s orders, lab requests, or follow-up needs with patients.
  • Performs proper treatments as ordered by and under the supervision of the physician or nurse practitioner.
  • Documents services performed and patient assessment findings.
  • Dresses wounds, apply compresses, ice bags, and hot water bottles.
  • Observe patients and reports adverse reactions to medication or treatment to medical personnel in charge.
  • Administers specified medication, orally or by subcutaneous or intermuscular injection, and notes time and amount on patients’ charts.
  • Initiates emergency resuscitation measures according to protocol.
  • Reports patient condition to appropriate personnel during each shift while maintaining patient confidentiality.
  • Assembles and uses such equipment.
  • Collects samples and labels specimens, such as urine, blood, and sputum, from patients for testing and performs routine laboratory tests on samples.
  • Sterilizes or cleans equipment and supplies.
  • Cleans rooms according to the protocol after each patient and at the end of the day.
  • Helps with administrative tasks such as answering patients’ calls, texts, e-mails, and scheduling appointments while maintaining patient confidentiality.
  • Maintains supplies, including stocking and ordering new supplies and disposing of expired or damaged supplies.
  • Responds to patient inquiries by evaluating and responding to the patient’s problems and questions.
  • Confirms appointments and follows up with patients who miss appointments.
  • Follows up with patients on lab results, including the scheduling of any other tests.
  • Coordinates referrals for patients through insurance and other physician offices.

Education and/or Experience

Graduate of an accredited LPN/LVN program and one to two years related experience and/or training; or equivalent combination of education and experience. Possess thorough working knowledge of anatomy, general medical conditions, general therapies, and interventions.

Language Skills

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

Mathematical Skills

Ability to add, subtract, multiply, and divide into all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

Reasoning Ability

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

Computer Skills

To perform this job successfully, an individual should have knowledge of Surepoint Family Practice’s information technology and systems. The platform includes but is not limited to Athena.

 

Certificates, Licenses, Registrations

  • Current licensure in good standing in the State of Texas.
  • BLS certified.
